Papa’s Post-Discharge Approach

Upon hospital discharge, members are introduced to Papa to support them through their transition and recovery process. This includes:

Providing valuable social support to ensure members can recover at home and adhere to discharge plans

  • Provide companionship and emotional support to ease recovery
  • Assist in the home and with other health-related social needs that arise
  • Provide transportation to and from appointments, prescription and medical supply pickups, and grocery shopping

Boots on the ground to identify and prevent crisis-driven care

  • Motivate and help members follow through on follow-up appointments and care
  • Identify and report potential health deterioration
  • Act as eyes and ears in the home and provide regular updates on the member’s status and needs

How it works

  1. Member referral/notification

    Health plan provides Papa with post-discharge membership through standard reporting/care management referral process.

  2. Papa engagement

    Papa team outreaches members within 48 hours to schedule in-home support and address identified needs. Members have access to Papa benefit hours over the post-discharge period.

  3. Meet member needs

    Papa Pals support members in the home, with transportation, prescription pickup, access to care, or other health-related social needs. If Papa Pals identify a member need they cannot address, that information is escalated to the health plan for their care team to address.

  4. Report outcomes

    Papa reports on activities delivered and health plans report back on claims-based outcome data to measure results.
